SCAF SWIMMING 2024/25 REGISTRATION Information Sheet

Attached is the Registration Form for the SCAF 2024/25 swim season. It is a fillable pdf including your agreement to the Codes of Ethics (three separate acknowledgments). Please fill out the form completely on your computer including typed name and date on Codes of Ethics. Hand written forms will not be accepted. Computer Note: Do not work in “Preview.” Drag form to your desk top, compete and save then send to Virgis.

Note Area Association: You may select ONLY your Primary Area.

Please select “Yes” or “No” check box if you are a USAS swimming official. If

“Yes,” please check your highest certification level.

 

On the final page, there is a button to SAVE the form to your computer. When you press SAVE, form will check to make sure that all REQUIRED fields have been filled out. Form will not save until fields are complete.

 

When file is saved, you have 2 options:

 

Option 1 PREFERRED:
A. Forward your saved form to SCAF Membership Chair, Virgis Trasikis at

vtrasiki@sprynet.com

1. Use Zelle to pay your $65 dues. Payment by Zelle is to: TreasurerSCAF@gmail.com

2. Payment should be received within 48 hours of submitting the membership form, or the registration form may be deleted.

 

Option 2:
A. Print out the saved form
B. Pay dues by check to SCAF ($65) C. Mail form and check to:
